This form is used to allow international students aged under 18 to officially request a change of accommodation from the current CAAW approved accommodation.

This form is to be completed by the caregiver from the approved Welfare provider on behalf of a parent. This form is in accordance with Standard 5.3.5 of The National Code 2018.

All approvals from ACU will be provided by email.
