This form is used to request a temporary accommodation and welfare waive for international students aged under 18 (onshore).

This temporary accommodation and welfare waive is intended to allow international students aged under 18 (onshore) to stay with parent(s) visiting Australia for a set period of time instead of staying with the CAAW approved accommodation under the care of the approved welfare provider.

This form is to be completed by a parent, or the approved caregiver from the approved welfare provider on behalf of a parent. This form is in accordance with Standard 5.3.5 of The National Code 2018.

All approvals from ACU will be provided by email.

As parent(s) of the above student, I/we wish for my child to stay with the following parent when visiting Australia. I declare that I will take full responsibility for my child's welfare and agree to temporarily waive the accommodation and welfare arrangements for the proposed dates below.
